I once had a client who used an ATA as their router.  Basically modem in
bridge mode connected to the ATA which had one ethernet port connected
to his one computer so the phone actually had the external IP address.
He could talk to anyone except me.  Turned out that my server recognized
that he was not behind a NAT so it connected directly.  Problem was, I
was behind a NAT so it didn't work.

I solved it by turning off the ability to connect the calls directly but
it would be great if it was able to recognize when both phones were on
public IPs.

Hugh's dream could come true.  All we need to do is adopt IPV6 and give
every phone a public IP address.  We could do away with the proxy server
(what I do) and make direct calls just like making SMTP, ssh calls and
the like.  We could also do port forwards everywhere but that would only
allow for one phone in an office.

That would also be asking for too much network knowledge from your
average luser.
